Evaluations
Midterm Evaluation
All students participate in an onsite or virtual visit with a Program Advisor (PA) or Employer Relationship Manager (ERM) at least once during a 4 or 8 month work term. During the midterm evaluation, your PA or ERM will complete an evaluation form documenting your title, location, supervisor, and the tasks you are performing in consultation with your supervisor. Your PA or ERM will discuss the evaluation form with you. It should provide you with useful feedback, both appreciative and constructive.
Purpose of the midterm evaluation
- Enhance the understanding of the actual duties of the student and of the work environment.
- Familiarize supervisors with our evaluation process, and receive information regarding your performance on the job.
- Evaluate the usefulness and success of the placement.
- Encourage supervisors to make continued use of the Co-op Program.
- Offer assistance to students and supervisors with any aspect of the placement.
- Discuss with students their learning opportunities and professional development in the work environment.
- Assist students in the further clarification of their career objectives in light of the experience of the placement.
- Allow students the opportunity to express their feelings regarding the job.
- Ensure that students are fully aware of available university, faculty, and co-op support, including who to contact and in what circumstances, how to reach out, and when to seek assistance.
In some circumstances, a second midterm evaluation may be required either during the same four-month work term or during the consecutive four-month work term with the same employer for which a midterm evaluation is not curricularly required. For example:
- A student's duties change radically during a work term.
- A student requests and justifies the need for a second visit.
- A supervisor requests the need for a second visit.
- A placement is, in the judgment of the Co-op Office, less than acceptable.
End of Term Employer Evaluation
An End of Term Employer Evaluation (EoTEE) is required for each 4-month work term in order for a student to obtain credit. More importantly, feedback is important to the student’s development and future success in engineering.
The Process
Supervisors will receive a unique campusBRIDGE link via email to complete the evaluation. Emails will be sent no later than three weeks prior to the end of each 4-month work term.
Important: You do not require a campusBRIDGE account in order to complete the form, but you will need to use Chrome, Firefox, or Safari as your browser. If the evaluation form is left open for 30 minutes it will time-out and any information entered will be lost. Be sure to intermittently scroll to the bottom and click the ‘Save as Draft’ button to ensure no data loss.
Issues with the campusBRIDGE link?
If your supervisor is unable to use the link to the evaluation form in campusBRIDGE, please have them complete this fillable PDF form. Once the form is completed, you and your supervisor will have a discussion about it. Afterwards, either you or your supervisor will:
- Save the completed evaluation form using the following naming convention: ID#_EREV (e.g., 1234567_EREV).
- Email the signed PDF to the co-op team member that conducted the onsite evaluation, copying you, by no later than the last day of the current 4-month work term.
Deadlines
Students’ employer evaluations are due on the last day of the work term, April 30, August 31, or December 31. Students should ensure that their supervisor completes and forwards these evaluations to the Co-op Office on time. When evaluations are not received on time, the procedure will be as follows:
- The student contacts their supervisor to remind them that an evaluation needs to be completed.
- If the above is not successful, the student should contact the Co-op Office. A co-op staff member will contact the supervisor to obtain either the written evaluation or a verbal assessment of the student's performance.
- If the employer evaluation is impossible to obtain, a student's grade shall be assigned based on the midterm evaluation and work term report.

Appeal Procedure
Students may appeal academic decisions relating to their status in the Co-op Program through the existing faculty and university appeal processes. For support in this process, you may contact the Office of the Student Ombuds.

How do I set up the onsite visit?
Midway through the second month of your work term (about 6-7 weeks into the term) you will be contacted by a co-op staff member to arrange potential dates for a meeting. See the Work Term Checklist in the for more information.
